SPORTS PERFORMANCE: Pre-Participation Medical Examinations
Prior to arriving on campus, all incoming student-athletes are required to complete a set of medical forms that will be available on JumpForward. These forms are mandatory and failure to complete them by the assigned deadline will delay clearance for participation in team activities. After submission, members of the athletic training staff will review and approve all medical forms. Once on campus, all incoming student-athletes must pass a physical completed by a team physician in order to participate in team activities. The pre participation medical exam will consist of general medical and orthopedic exams, as well as a mental health screening administered by a qualified mental health professional.
Members of the athletic training staff and team coaches will directly communicate information on how to access online forms and details about the date, time and location of sports physicals. No member of the University of Pennsylvania’s athletic training staff will permit a student-athlete to participate, nor will the University of Pennsylvania provide insurance coverage to any student-athlete who has not completed and passed the pre-participation physical examination process.
